Two Forms of ID. There are Absolutely No Exceptions To These Requirements. If Adrresses are on both forms of ID’s - the addresses must match

▪ Valid State Non-Driver’s Identification Card

▪ Armed Forces Card

▪ Government Issued ID

▪ University Issued ID (Student or Faculty)

▪ Recognized Corporate ID (No Membership Cards)

▪ Alien Registration Card

▪ Certification of Naturalization

▪ Current Lease, Mortgage, or Deed of Trust

▪ Voter Registration Card

▪ Home or Vehicle Insurance Policy

Unacceptable Identification

Acceptable Identification - TWO ARE REQUIRED AND ONE MUST BE A PHOTO ID
EVERY ADULT RECEIVING MAIL WILL NEED TO COMPLETE AND SIGN THE FOLLOWING:
